Adult Disabilities Services- YG
What: Adult's Disability Services provides support for adults 19 years of age or older living with intellectual or developmental disabilities. We offer:
case management with a focus to promote inclusion in the community;
respite funding for families caring for loved ones;
community supports such as day programs; and
a range of supported living services
Who: To be eligible for ADS services, applicants must:
1) Be 19 years of age or older
2) Be a Yukon resident (live in the Yukon for 3 months)
3) Have an Intellectual Developmental Disorder (Intellectual Disability)* or equivalent intellectual and functional limitations, existing before the age of 19
4) Apply voluntarily or with the support of a legal guardian

Adult Protection and Decision-Making- H&SS Yukon
What: Designated agency mandated to investigate all reports of adults aged 19 and over in the Yukon who may be abused or neglected, including older adults and adults with a physical or intellectual disability who are unable to seek help for themselves. Social workers work with community responders to ensure that an intervention is not intrusive and is guided by the adult's values and beliefs. Only in circumstances where it appears that the adult is not capable of obtaining help will a more formal inquiry proceed, under authority of the Adult Protection and Decision-Making Act. Reports can be made anonymously and the act also protects the identity of a person making a report. Also provides a range of tools to assist adults who have some diminished ability to make their own decisions. Tools include Supported Decision-Making Agreements, Representation Agreements, court-appointed guardianship, and adult protection.
What are abuse, neglect and self-neglect?
Abuse is when someone’s actions cause harm or injury to another person. This can be physical, emotional, mental, or financial.
Neglect is when someone does not provide the necessary care or support a person needs.
Self-neglect is when someone cannot care for their own basic needs.
Signs of abuse or neglect
Isolation is the biggest risk factor.
Some signs include:
fear, anxiety or depression;
avoiding friends or activities the adult usually enjoyed;
unexplained injuries;
changes in personal hygiene or weight loss;
unsafe living conditions;
unpaid bills or sudden changes in spending; and
missing basic needs like food or shelter.

Children’s Disability Services- YG
What: Children’s Disability Services We promote inclusion of children with disabilities in community life. CDS has supports and funding available. Families will first need to access other appropriate resources prior to accessing the CDS program (eg.., private insurance, employee benifits programs). Our social workers help families access the services they need.
These services include:
Coordination, case managemnet, and transition planning with a Social Worker
Respit care- in home, and out-of-home;
Inclusion supports- to assist a child to participate in community and recreational activities;
If eligible, the following services may also be offered:
Family counselling - for parents/guardians requiring professional counselling to strengthen your ability to care for and face the challenges of caring for your child's disability;
Sibling care - when a parent/guardian must attend a medical appointment or therapy with their child and another family member is not able to take care of the sibling;
Ocupational Therapy and/or Physiotherapy

Applied Behaviour Analysis - a theraputic intervention for children with Autism Spectrum Disorders
Specialized camps
Homemaker services for the family - including routine housekeeping and laundry services;
In-home child care where, because of the child's complex needs, this is the most appropriate and cost-effective way to provide for the child's and family's needs.
Who: You're eligible if:
you're a Yukon resident;
your child is under 19 years of age; and
your child has a chronic develpomental, physical, sensory, cognitive, mental health or neurological impairment that significantly limits a child's ability to function in normal daily living. A child does not necessarily need a diagnosis and can be in the process of assesment to recieve supports.

Developmental Diagnostic and Support Clinic
What: The Developmental Diagnostic and Support Clinic is part of Disability Services. The clinic offers:
Autism Spectrum Disorder (ASD) assessments and diagnosis for school-aged children;
Fetal Alcohol Spectrum Disorder (FASD) assessments and diagnosis for adults.

Exercise Services- YHLP
What: Exercise services are available to all Yukoners living with 1 or more chronic conditions. After an initial assessment, the clinical exercise specialist will:
recommend which exercise services are available; and
outline strategies that’ll help improve your fitness and overall health.
This service includes:
risk screening and activity plan development (summaries are shared with your primary care provider);
supervised group exercise classes (available at the Canada Games Centre and at the chronic conditions program facility’s gym);
virtual exercise classes; and
exercise routine development along with education and recommendations on how to stay safe while being active and exercising.
Who: Yukoners with chronic conditions 18 and up.

Finding a Doctor
What: The Find a primary care provider service connects residents of Whitehorse and surrounding areas with primary care providers who are accepting new patients.
Who: To use this service, you must:
have a valid Yukon Health Care Insurance Plan (YHCIP);
be 16 years old or older; and
not have a family doctor or nurse practitioner.

What: Referred Care Clinic
Who: Our Referred Care Clinic is a primary care clinic for vulnerable adults with:
unstable mental health; or
substance use disorders.
We serve people who:
do not have a primary care provider, such as a doctor; and
rely on outreach services.
How do you access this service?
Our Referred Care Clinic only accepts referrals from:
Mental Wellness and Substance Use Services nurses;
the Whitehorse General Hospital’s attending physician;
psychiatrists in the Yukon; and
overdose prevention coordinator (oopc@yukon.ca).
All referrals will be assessed for appropriateness by the Referred Care Clinic's lead physician and nurse practitioner.

What: The Whitehorse Walk-In Clinic is a health care facility that provides access to same-day, non-urgent care for all Yukon residents, with or without a family doctor. It can support you:
with minor health concerns that do not require long-term follow-up; or
as an alternative to visiting the Emergency Department for issues that do not require emergency care.
Who: The walk-in clinic offers care to Yukoners covered by the Yukon Health Care Insurance Plan. The clinic serves patients with Canadian health care coverage and a provincial or territorial health card.
To have insurance under the Yukon Health Care Insurance Plan, you must:
be a Canadian citizen or have the necessary immigration status;
make the Yukon your permanent home;
be present in the Yukon for the majority of the year, or have a waiver from Insured Health Services if you're absent for more than 6 months.
New residents

Health Equipment Loan Program- Red Cross
What: The Canadian Red Cross Health Equipment Loan Program (HELP), provides health equipment to individuals recovering from illness or injury, and empowers people to live with independence and dignity. Like a prescription, healthcare professionals refer clients to the Canadian Red Cross and health equipment is selected to safely meet the needs of each individual.
HELP is a volunteer-supported, community-based service made possible through the support of, provincial governments, health authorities, donations of used equipment, donor contributions, and the efforts of thousands of volunteers and professional staff.

What: Whitehorse services include:
